Insurance ID Cards and UT FLEX Debit Cards for the 2012-2013 Plan Year
Enrollees in the new dental plans (UT SELECT Dental Plus and DeltaCare USA Dental HMO) and the new Vision Plus plan will receive new insurance identification cards before the start of the 2012-2013 Plan Year (which begins on September 1, 2012). Please see the table below for more information.
To order additional ID cards for your dependents, please check the plan website for options or contact the customer service team after September 1, 2012.
