New! Dental HMO – DeltaCare USA

As the result of a recent Request for Proposal and competitive bid process, the fully insured dental HMO option formerly provided by Assurant Employee Benefits will now be provided by Delta Dental as DeltaCare USA. The new DeltaCare plan has reduced monthly premium rates while providing the same features of the previous dental HMO plan with set copayments, and no annual deductibles or maximums for covered benefits. In addition, the DeltaCare plan offers an expanded service area to include the Tyler area and part of the Valley.

DeltaCare Premium Rates 2012 - 2013

Enrollment Tier Premium (per month) Decrease from Current Premium
Employee $8.55 $1.50
Employee & Spouse $16.25 $2.85
Employee & Child(ren) $17.96 $3.15
Employee & Family $25.65 $4.50

Some added features with DeltaCare are fewer frequency limits on some services, and lab fees included in the copayment schedule so you know your costs up front. Plus, if you or one of your covered dependents is in orthodontic treatment under the Assurant dental HMO or other dental insurance, DeltaCare allows you to continue the treatment with your current dental provider (regardless of their network status) with the same copayments and coverage of the previous plan.

If you are currently enrolled in the dental HMO plan under Assurant and wish to continue to be enrolled in the dental HMO plan with DeltaCare, your coverage will be automatically transitioned from Assurant to DeltaCare effective September 1, 2012. There are some differences in network providers so you may need to choose a new dentist. If your current Assurant dentist participates in the DeltaCare network, your provider selection will remain in place unless you wish to change your selection. If your current Assurant dentist does NOT participate in the DeltaCare network, you must select a new dentist in My UT Benefits during Annual Enrollment or one will be selected for you based on your address. Your coverage option letter will alert you if you need to select a dentist.

To find out if your current dentist is in the DeltaCare network, you may call Delta Dental at (800)893-3582.
