The final column of "Additional Information" highlights details such as the Summary of Benefits and Coverage (SBC), Evidence of Insurability (EOI) requirements, and a brief description of the Tobacco Premium Program.

UT SELECT Medical (BCBSTX) and Prescription Drug Plan (Express Scripts) Slight premium increase. New $100 copay for MRI and CT scans; new out-of-pocket maximum to include medical copays; UTSW Network pilot program concludes on August 31, 2014. No EOI required. Summary of Benefits and Coverage (SBC) available July 15. New ID Cards will be issued.
Tobacco Premium Program (TPP) Monthly cost billed with UT SELECT Medical premium. Applies to tobacco user(s) (16 and over) enrolled in the UT SELECT Medical plan. Monthly cost based on three categories: Member $30, Spouse $30, and Child(ren) $30. Maximum of $90 per family per month for three or more tobacco users.
