Insurance ID Cards and UT FLEX Debit Cards for the 2011-2012 Plan Year
Insurance ID Cards
Because there are no changes to key benefits information such as deductibles, copayments, and customer service contact information for the UT SELECT Medical (including prescription), UT SELECT Dental, Assurant Dental, or Superior Vision plans for the upcoming 2011-2012 Plan Year, new ID Cards will not be issued this year to continuing participants in these plans. If you have newly enrolled in medical, dental, or vision coverage, you should receive your new ID Card(s) by September 1, 2011.
For those who have added family members to coverage on one or more of these plans and need additional ID Cards after September 1, 2011, you may either check the plan website for options or contact the customer service team for each plan and request that additional cards be issued. Please remember that you should not discard any of your UT Benefits Insurance ID Cards or your UT FLEX Debit Card until you have either received a replacement card or discontinued your participation in the plan.
PayFlex Debit Cards
Please Note: New “HealthHub Powered by PayFlex” branding will be used for PayFlex Debit Cards issued for the upcoming plan year.
If you do not have a PayFlex Debit Card for the current plan year and you elected a Debit Card for your UT FLEX Health Care Reimbursement Account (HCRA) for the 2011-2012 Plan Year, you can expect to receive your new Debit Card by September 1, 2011.
If you currently have a PayFlex Debit Card and elected to continue the Debit Card for your UT FLEX HCRA for 2011-2012, you should check the expiration date on your current card. If the card does not expire in August 2011, you should keep it and your new election will be available through your current Debit Card on September 1, 2011. If your Debit Card expires as of August 2011, you will receive a replacement Debit Card prior to September 1, 2011.
