Adding a Subscriber

When you add a subscriber into your eligibility system, SGELIG will be updated through an automated process once EGI receives your component’s next eligibility dataset. EGI will, in turn, send that new information to each appropriate vendor on the next scheduled transmission day for those vendors.

At times, you may want or need to get the subscriber’s information into SGELIG more quickly, for example, a newly hired or rehired employee has a doctor’s appointment in the next few days or needs a prescription filled immediately. In this case, you will not only need to update your component’s eligibility system, but also update SGELIG manually online. All eligibility maintenance is done through either the '111' in *SGELIG mainframe, or using the Add Subscriber function in the View/Change menu of SGELIG on the web. Information required to add a Subscriber in SGELIG.

Date format: All dates must be entered in YYYYMMDD format.

Mainframe Field level help: Each input field offers field level help which provides information concerning what data should be entered.

PF keys: Several helpful functions offered in *SGELIG mainframe are accessed through the PF keys. PF1 lists these keys and functions.

Access the 111 Screen

Type ‘111’ in the command line, a space and the Subscriber's SSN. Press ‘Enter.’

If the Subscriber worked for your component at another time, *SGELIG will access the information from the previous employment period. You will be able to change the older information as necessary while using the Add Subscriber function.

If the Subscriber did not previously work for your component, the message 'No Record Found' will appear in the upper left corner of the screen. In the command line, type '111' and a period. This should bring you to a blank 111 screen.

Access the Add Subscriber function screens

Type ‘ADDSUB’ in the ACTION Command line located in the center of the 111 Screen. Press 'Enter.'

INSURED Screen

Enter the Subscriber's SSN in the SUBSCRIBER REFERENCE/SSN field

If the Subscriber is an ACTIVE employee, leave the '01' in the RELATIONSHIP CODE field.

If the Subscriber is NOT an ACTIVE employee, i.e. he/she is a retiree or a surviving spouse, access field level help to find the relationship code that describes the Subscriber's situation.

Leave the remaining fields blank.

Press 'Enter.'

BIOGRAPHICAL Screen

Enter the Subscriber's name in the appropriate fields.
First and Last name are required. Do not use prefixes such as Mr., Mrs. or Dr. in the first name field. Do not use a '.' after a middle initial.
Suffix includes identifiers such as Jr., III, PhD and Esq.

Enter the Subscriber's date of birth in YYYYMMDD format.

Enter Subscriber's gender, M or F.

If the Subscriber's US citizenship status is available, enter a 'Y' for 'Yes, a US citizen,' an 'N' for 'No, not a US citizen,' or leave blank if citizenship is unknowm.

Press 'Enter.'

DEMOGRAPHIC Screen

Enter the Subscriber's address information in the appropriate spaces. Street address, city, state and zip code are required for domestic addresses.

If the Subscriber's address is foreign, type a 'Y' in the blank next to the question 'IS THIS A FOREIGN ADDRESS?' and zip code will not be required.

If the street address is longer than the field immediately to the right of the word ADDRESS, use the second address field located directly below the first one.

Enter the Subscriber's phone number if available. Do not use '(' ')' or '-' when entering the phone number.

Answer the questions by typing a 'Y' for 'Yes' in the blank to the right of the question, or by leaving blank for 'No.'

IS THIS A PRIVATE ADDRESS?

Answer yes, 'Y,' if the Subscriber has requested his/her address to be kept private and protected from public record requests.

IS THIS A PRIVATE PHONE?

Answer yes, 'Y,' if the Subscriber has requested his/her phone number to be kept private and protected from public record requests.

EMPLOYMENT Screen

If the Subscriber is an ACTIVE employee, leave the 'ACT' in the ELIGIBILITY STATUS field.

If the Subscriber is NOT an ACTIVE employee, i.e. he/she is a retiree or a surviving spouse, access field level help to find the status that describes the Subscriber's situation.

Check the benefit and employment effective dates for accuracy. If the dates are incorrect, enter the correct dates in these fields in YYYYMMDD format.

If any of the following information is available, enter it in the appropriate blank(s): JOB CLASS CODE, STATE SERVICE MONTHS, CAMPUS MAIL CODE, WORK PHONE/EXT, FAX NUMBER, E-MAIL, EAM CODE, EEO CODE, FLSA EXEMPT.

Press 'Enter.'

MEDICAL Screen

If the Subscriber has made NO selections or changes to his/her coverage ( default coverage ), follow the directions for the Default Coverage Option .

If the Subscriber has made ANY selections or changes to his/her coverage ( selected coverage ), follow the directions for the Selected Coverage Option .

If the Subscriber has waived his/her medical and dental coverage ( waived coverage ), follow the directions for the Waived Coverage Option .

If the Subscriber has declined coverages paid by premium sharing dollars ( declined coverage ), follow the directions for the Declined Coverage Option .

 DENTAL Screen

If the Subscriber has made ANY selections or changes to his/her coverage ( selected coverage ), follow the directions for the Selected Coverage Option .

If the Subscriber has waived his/her medical and dental coverage ( waived coverage ), follow the directions for the Waived Coverage Option .

If the Subscriber has declined coverages paid by premium sharing dollars ( declined coverage ), follow the directions for the Declined Coverage Option .

 VISION Screen

If the Subscriber has elected Vision coverage, follow the steps listed below. If the Subscriber has not elected Vision coverage, skip to the 'No Vision coverage' section below.

  1. Leave the ACTION CODE as 'NW' for a new Subscriber.
  2. Type the vision coverage plan code in the PLAN blank. If you don't know the valid vision coverage plan codes for the Subscriber's zip code, access field level help to find the valid vision plan codes.
  3. Type the code for the level of coverage the Subscriber has selected in the LEVEL field. If you do not know the valid level codes, access field level help to find them.
  4. Press 'Enter.'

Proceed to the AD & D section.

No Vision coverage

  • Remove the 'NW' from the ACTION CODE field.
  • Remove the date from the EFFECTIVE DATE field.
  • Press 'Enter.'
  • AD & D Screen

    If the Subscriber has elected PAI (AD&D) coverage, follow the steps listed below.

    If the Subscriber has not elected PAI (AD&D) coverage, skip to the 'No PAI (AD&D) Coverage' section below.

    1. Type the appropriate Selection Code in the SELECTION CODE field.
    2. Leave the ACTION CODE as 'NW' for a new Subscriber.
    3. Type the PAI (AD&D) coverage plan code in the PLAN field. If you don't know the valid PAI (AD&D) coverage plan codes for the Subscriber's zip code, access field level help to find the valid PAI (AD&D) plan codes.
    4. Type the code for the level of coverage the Subscriber has selected in the LEVEL field. If you do not know the valid level codes, access field level help to find them.
    5. If the Subscriber has elected a specific coverage amount, enter that amount in the COVERAGE AMOUNT field. Leave the COVERAGE MAXIMUM field blank. If the Subscriber has not elected additional PAI (AD&D) coverage, enter the default coverage amount of $10,000.

      If the Subscriber has elected the maximum available coverage, leave the COVERAGE AMOUNT field blank and type a 'Y' in the COVERAGE MAXIMUM field.

    6. Enter the actual annual salary of the Subscriber in the ANNUAL PAY RATE field. If the Subscriber's annual salary is not available, you may leave this field blank.
    7. Press 'Enter'

    No PAI (AD&D) Coverage

    If the Subscriber does not want any PAI (AD&D) coverage:

    1. Type a 'D' for declined this coverage in the SELECTION CODE field.
    2. Remove the 'NW' from the ACTION CODE field.
    3. Remove the date from the EFFECTIVE DATE field.
    4. Press 'Enter.'

     LIFE INSurance Screen

    If the Subscriber has elected Life coverage, follow the steps listed below.

    If the Subscriber has not elected Life coverage, skip to the 'No Life Coverage' section below.

    1. Type the appropriate Selection Code in the SELECTION CODE field.
    2. Leave the ACTION CODE as 'NW' for a new Subscriber
    3. Type the Life coverage plan code in the PLAN field. If you don't know the valid Life coverage plan codes for the Subscriber's zip code, access field level help to find the valid Life plan codes.
    4. Type the code for the level of coverage the Subscriber has selected in the LEVEL field. If you do not know the valid level codes, access field level help to find them.
    5. Enter the coverage amount the Subscriber has selected in the Life COVERAGE AMOUNT field.
    6. Enter the actual annual salary of the Subscriber in the ANNUAL PAY RATE field. If the Subscriber's annual salary is not available, you may leave this field blank.
    7. Press 'Enter'

    No Life Coverage

    If the Subscriber has waived or declined life coverage:

    1. Type a 'D' for declined this coverage in the SELECTION CODE field.
    2. Remove the 'NW' from the ACTION CODE field.
    3. Remove the date from the EFFECTIVE DATE field.
    4. Press 'Enter.'

     LONG TERM DISability Screen

    If the Subscriber has elected Long Term Disability (LTD) coverage, follow the steps listed below.

    If the Subscriber has not elected LTD coverage, skip to the 'No LTD Coverage' section below.

    1. Type the appropriate Selection Code in the SELECTION CODE field.
    2. Leave the ACTION CODE as 'NW' for a new Subscriber.
    3. Type the Long Term Disability coverage plan code in the PLAN field. If you don't know the valid Long Term Disability coverage plan codes for the Subscriber's zip code, access field level help to find the valid LTD plan codes.
    4. Enter the actual annual salary of the Subscriber in the ANNUAL PAY RATE field. If the Subscriber's annual salary is not available, you may leave this field blank.
    5. If the Subscriber selects the Pre-Tax option, type a 'P' in the TAX PREFERENCE field. If he/she has selected the After-Tax option, type an 'A' in this field. If the Subscriber's tax preference is not available, you may leave this field blank.
    6. Press 'Enter.'

    Continue to the FLEX dependent section.

    No LTD Coverage

    If the Subscriber has not elected LTD coverage:

    1. Remove any code that is in the SELECTION CODE field.
    2. Remove the 'NW' from the ACTION CODE field.
    3. Remove the date from the EFFECTIVE DATE field.
    4. Press 'Enter.'

     FLEX DEPENDENT DAYCARE Screen

    If the Subscriber has not elected the FLEX Dependent Daycare option, press 'Enter' and continue to the FLEX Health section.

    If the Subscriber has elected the FLEX Dependent Daycare option, complete the following fields.

    1. Type the begin effective date for the FLEX coverage in the COVERAGE EFFECTIVE DATE field. This date must be entered in YYYYMMDD format .
    2. Leave the COVERAGE TERMINATION DATE field blank.
    3. Type 'EN' for EMPLOYEE STATUS - NEW HIRE in the ACTION REASON field.
    4. Enter the annual amount the Subscriber has elected to contribute to his/her FLEX Dependent Daycare fund in the ANNUAL AMOUNT field.
    5. Enter the monthly contribution elected by the Subscriber in the MONTHLY AMOUNT field.
    6. Type the number of months the Subscriber is employed annually in the NUMBER OF MONTHS field.
    7. Press 'Enter.'

     FLEX HEALTHCARE Screen

    If the Subscriber has not elected the FLEX Health option, press 'Enter.'

    If the Subscriber has elected the FLEX Healthcare option, complete the following fields.

    1. Type the begin effective date for the Flex coverage in the COVERAGE EFFECTIVE DATE field. This date must be entered in YYYYMMDD format .
    2. Leave the COVERAGE TERMINATION DATE field blank.
    3. Type 'EN' for EMPLOYEE STATUS - NEW HIRE in the ACTION REASON field.
    4. Enter the annual amount the Subscriber has elected to contribute to his/her FLEX Healthcare fund in the ANNUAL AMOUNT field.
    5. Enter the monthly contribution elected by the Subscriber in the MONTHLY AMOUNT field.
    6. Type the number of months the Subscriber is employed annually in the NUMBER OF MONTHS field.
    7. Press 'Enter.'

    Confirm Add

    Type 'ADDSUB' when prompted.
