Please complete an Equipment Transfer Form. All CPU’s hard drives must be reformatted by OTIS and signed off on the form. When all parts of the form are complete, submit a work request requesting pickup, listing the equipment and the location.
Note: The items cannot be removed unless completed paperwork is attached.
