Please complete an Equipment Transfer Form. All CPU’s hard drives must be reformatted by OTIS and signed off on the form. When all parts of the form are complete, submit an SOS requesting pickup, listing the equipment and the location.
Note: The items cannot be removed unless completed paperwork is attached.
