to publish information on U.T. System Web servers. This form replaces
any forms currently on file so include the names of everyone authorized
to publish. Please forward completed forms to The Office of Technology and Information Services (TIS) CLB 5.104

Statement of Responsibility --
The U.T. System
Administration Internet Policy states, “Department heads are responsible
for approval and maintenance of information published by department employees
on U.T. System Administration Web servers.” Only those employees
who have signed a user acknowledgement as required by The
University of Texas System Policy for the Use and Protection of Information
Resources (BPM-53), and who have received instruction regarding
web publishing procedures, including training in the proper application
of copyright and libel law as it pertains to web publishing should be
authorized to publish. The required employee review materials are available
