Personal Travel to Foreign Adversary Country Form

The Personal Travel to Foreign Adversary Country Form is required for UT System Administration employees who travel for personal reasons to “foreign adversary” countries.  The form includes both a pre-travel notification which is to be provided prior to departure and a post-travel briefing to be completed upon return.
