Life Insurance Portability

If you are enrolled in voluntary term life coverage, you and your covered dependents may elect to continue coverage after leaving the UT System through portability. You and your spouse (if covered) must have been insured for at least one year. Portability is not an individual policy and does not have a cash value. Portability rates increase in five-year increments as you age. This coverage may continue until you or your spouse reach age 65 or your spouse or dependent children no longer meet the definition of a spouse or dependent child. You and your covered dependents are not eligible for portability if you have elected conversion or if you retire from the UT System.

To apply for portability:

  1. Determine your premium by using the Portability Premium Calculation Worksheet (PDF)
  2. Complete Part 2 and submit the Application for Portability (PDF) to grp-hrsp@utsystem.edu within 31 calendar days of the date you lose coverage.
  3. An OTI Service Partner will complete Part 1 and return the application to you.
  4. Mail your application and payment directly to the insurance company.

Life Insurance Conversion

If any portion of life insurance terminates, employees and their covered dependents may convert some of their basic life and/or voluntary life insurance to an individual whole life policy by remitting premium directly to Dearborn National. Individuals must have been insured under the policy for at least five years before applying for conversion. Insureds who elect to port coverage (portability) are not eligible to convert (conversion). As long as premiums are paid on time, coverage will continue until the insured submits a request to terminate coverage. Life insurance policies may be converted regardless of your current health and even if you retire from UT System.

To apply for life insurance conversion:

  1. Determine your premium using the Premium Calculation Worksheet (PDF).
  2. Complete Part 2 and submit the Application to Convert Group Term Life Insurance (PDF) to hrsp@utsystem.edu within 31 calendar days of the date your UT coverage ends.
  3. An OTI Service Partner will complete Part 1 and return the application to you.
  4. Mail your application along with payment directly to the insurance company.

Long-Term Disability Conversion

If you are enrolled in long-term disability (LTD) insurance, you may be able to convert the coverage to an individual policy. You are not eligible to continue LTD coverage if you retire from UT System, you have not been insured for at least 12 consecutive months under the policy or you are disabled under the terms of the policy. To convert your LTD:

  1. Determine your premium using the LTD Conversion Calculator provided by Dearborn National.
  2. Review the Conversion Enrollment Kit for additional details and submit the Employer Questionnaire (PDF) and your completed Conversion Application (PDF) to hrsp@utsystem.edu within 31 days of the date you lose coverage.
  3. An OTI Service Partner will complete the Employer Questionnaire and return the application to you.
  4. Mail your application and employer questionnaire along with payment directly to the insurance company.

Long-Term Care Portability

If you are enrolled in long-term care insurance, you and your spouse may continue coverage through direct home billing. Other covered family members also may continue their direct home billing. Direct home billing will not change your premium rate. To avoid cancellation of coverage, please contact the insurance company (can) at 1-888-825-0353 within 60 days of the date you lose coverage.
